Environmental Health & Safety asks that as a new researcher you introduce yourself to us by filling out a Principal Investigator Registration Form (EHS 6-1) so that we can determine your safety needs and what further involvement you will need from EH&S.

I am planning to do a research project involving one or more of the following, what do I need to do as far as EH&S is concerned?
- Etiologic/Select Agents
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1), the Select Agents Registration Form (EHS 7-1), and contact the Biological Safety Office at 850-644-5374.
If you have already registered with EH&S, you will need to fill out the Etio/Select Agents Registration Update Form (EHS 7-6) and contact the Biological Safety Office at 850-644-5374.
- Recombinant DNA
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1) and contact the Biological Safety Office at 850-644-5374.
If you have already registered with EH&S, you will need to fill out the Recombinant DNA Registration Update Form (EHS 7-7) and contact the Biological Safety Office at 850-644-5374.
- Human cell lines or blood
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1) and contact the Biological Safety Office at 850-644-5374.
If you have already registered with EH&S, you will need to contact the Biological Safety Office at 850-644-5374.
- Controlled Substances
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1), follow the steps outlined in Controlled Substance Licensing Information, and contact the Laboratory Safety Office at 850-644-8916.
If you have already registered with EH&S, you will need to follow the steps outlined in Controlled Substance Licensing Information and contact the Laboratory Safety Office at 850-644-8916.
- Radioactive materials
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1), fill out the Proposal to use Radioactive Materials (EHS 10-1), and contact the Radiation Safety Office at 850-644-8801.
If you have already registered with EH&S, you will need to fill out the Proposal to use Radioactive Materials (EHS 10-1) and contact the Radiation Safety Office at 850-644-8801.
- Hazardous Chemicals
If you have never registered with EH&S before, you will need to fill out the Principal Investigator Registration Form (EHS 6-1), attach a current Chemical Inventory (EHS 8-1), and contact the Chemical Safety Office at 850-644-7682.
If you have already registered with EH&S, you will need to send a current Chemical Inventory (EHS 8-1), and contact the Chemical Safety Office at 850-644-7682.

Yes. You need to enroll in the Medical Monitoring Program for Vertebrate Animal Users. To do so, fill out the Medical Monitoring for Vertebrate Animal Users Form (EHS 7-2) and contact the Biological Safety Office at 850-644-5374.
